At Loyola University New Orleans, it is our priority to make sure that your voice is heard and respected. You have several different options for reporting a concern with the university. This page outlines our process for reporting concerns and describes the formal and informal reporting options available depending on your situation.

* Discrimination and Harassment Complaints submitted via the bias incident form are informal in nature and their intended outcomes are educational, or restorative, in nature. If the student filing the complaint wishes to file a formal complaint that may result in disciplinary action taken against the faculty/staff/student whom the complaint is filed against, then the student making the complaint must file a formal complaint. In addition to the process outlined here, Title IX complaints are required to follow additional regulations provided by the federal government.

  • Title IX / Sexual Misconduct Report Form – Report alleged sexual misconduct, domestic or dating violence, stalking or retaliation pursuant to the Student Code of Conduct.

  • Academic Grievance Process – Submit a complaint in writing to an administrative officer of the university concerning an academic decision made by a person or group of persons acting in an official university capacity that directly and adversely affects the student.

  • ADA Grievance Reporting– If a student is dissatisfied with accommodations or decisions regarding a disability status, they may follow this process for filing a grievance. This page outlines the informal and formal dispute resolution procedures.

  • Formal Complaint Form – Report Code of Conduct violations and/or formal complaints of discrimination/harassment.

  • Bias Incident Reporting Form – Report an act of discrimination, harassment, intimidation, violence, or criminal offense committed against any person, group, or property that appears to be intentional and motivated by prejudice or bias. This is an informal report.
